Create a new document with a white background approximately 900px x 645px. Then create a new layer titled Header Bg. Using the Rectangular Marquee Tool, make a selection at the very top of the document that spans the entire width and is about 85 pixels high. Fill it in using #404040.

Next right-click on the current layer and select Blending Options. Create a Gradient Overlay.

Next we are going to create a 5px diagonal scanline to cover the header. To create this pattern, create a new 20px x 20px document with a transparent background. Draw a 1px line with your Pencil Tool from one corner of your canvas to the other. (See Ex 1 below.) Once you've done that, add lines next to it to make it thicker. To create a line 5px wide you need to add two rows of pixels on both sides of the 1px line already created. (1+2+2=5). If you create your pattern now, you will notice some gaps in your line. Although this might give you some interesting effects, that isn't what we want. To solve this, add two rows of pixels in both of the other corners.

Scanline Examples Now in order to use the pattern, you will need to define the pattern by going to Edit > Define Pattern. Give it a name that you will remember for future use.
